22 copies of the visa application form, thoroughly filled out and dated, bearing applicant’s signature.
32 identical passport size, front view, white background and recently-taken photographs.
4An official invitation should be sent from the Consular Department of the Ministry of Foreign Affairs of the Republic of Azerbaijan.
5For business, education and humanitarian visas: if an invitation from a receiving party or partner is not sent through Consular Department of the MFA, travelers should submit an invitation received by fax directly from a receiving party in Azerbaijan and a letter of request from the employer.
6The applicant must provide the exact phone number and address of the inviting party and also provide a copy of an Azerbaijani Citizen Identity Document, UNLESS an invitation is sent through the Consular Department of the MFA by the inviting party.
